This voucher is valid for 6 months. Please specify the name of the gift recipient in the "Order Notes" section of the Shop Check-out process.

Terms and Conditions
- This voucher is valid for a 30 minute appointment with the Oral Health Therapist.
- This voucher includes comprehensive examination, 2x standard intra-oral digital x-rays, intra-oral photography, home care recommendations, treatment planning and treatment costs (if required).
- This is valid for persons up to 17 years old. The voucher does not include any active dental treatment. Any additional treatment will be discussed and costs will be offered prior to proceeding.
- Some individuals may be able to claim a rebate from their health insurance. An itemised receipt will be issued at the appointment. This receipt may then be presented to the health insurer.
